Was that actually her therapist toying with the pen? Or was it simply something she was remembering, along with her recollections of her father, his death, and water? I'd made the assumption that recollections of her indoctrination techniques were surfacing in the form of dreams...

And yes, I also think TiKO is directly connected to the order.
